MEPS 

680-3      This is the form you will use to request your military Physical. Refer to this example 680-3 if needed.

DD-2807-2    Physical Qualification Pre-Screen. This is the document that you will fill out in order to complete your military physical. Refer to this DD-2807-2 Example if needed.

ADDITIONAL DOCUMENTS: Please fill out and send back.

Birth Certificate: Please provide a photocopy of your birth certificate

Certificate of Naturalization: Please provide a photocopy of your naturalization certificate if applicable.

Social Security Card: Please provide a photocopy of your social security card. Ensure that it is signed on the front!!!

Intent to Enlist Document: Please sign this document stating that you intend on accepting the Nuclear Engineering position should you be offered one during the interview in with the Admiral. The “Enlist” part is strictly for payment purposes as the Navy needed some way to classify payments to you while you are still in school and collecting a Navy paycheck.

SF-86 Background Investigation: This is for your security clearance and is filled out through this website link https://nasis.cnrc.navy.mil/ Your username will be emailed to you by your recruiter after your NVIP presentation(s).  You can login/out as you please while completing the form as long as you save your progress by clicking the save button at the bottom of the page. I have included a document that has some instructions for this and information to gather before you start it

Tattoo Screening Certificate: Please fill this out and email it back if you have any tattoos. Tattoos are not disqualifying for the NUPOC program in most cases, however, you should wait to get any more until you have finished school.

Statements of Understanding: This covers various topics regarding the Navy’s expectations for you while you are applying to and a member of the NUPOC program. Please initial at the end of each block of text and sign below the last one. Your recruiter will type in the rest of your information once you have emailed it back to them.

Disclosure of Social Security Number: This covers why the Navy will need your social security number during your application process. Your SSN will be used as a unique identifier for your application and will never be misused.

Commitment to Success: This covers various expectations and resources available to you prior to going to OCS/ODS. The OCS Delayed Entry Program (DEP) Guide to Success can be found here, and the physical fitness and nutrition guide may be found here. You will need to stay physically fit to remain in the NUPOC program.

Most of these documents contain personal information. As such, a link to transmit these documents to your recruiter with encryption can be provided at your request.
